The Future of Care Coordination in California: A Synthesis of the “Beyond the MSSP Waiver” Convening

May 2009 – Seniors can face confusing care options in their efforts to remain in their homes as they age. To support these efforts, the California Health and Human Services Agency (CHHS) and the California Department of Aging facilitate the coordination of medical, social and other community support services for seniors living at home through a range of programs. As the longest existing care coordination program for the elderly, the Multipurpose Senior Services Program (MSSP), is a critical part of this long-term care system.
